Older mh double bearing template

Trying to update my 2012 hm to the double bearing and looking for the template to drill the 4 holes. Any one have that template hiding in there tool box?

I laser cut a plate that has the holes to drill and bolts to the current holes. Could send u the dimensions or possibly send that.

If you would send the plate which then I could send it back to you that would be awesome. I would also like to pick your brain about swapping to the splined shafts, that info would be great.
