Yes, seen that many times over the years.
Root of problem is low torque on the primary clutch bolt. The torque on the clutch bolt reducing because the primary clutch bolt "Washer" has failed.
Note - if you have previously tightened the primary clutch bolt and as you are approaching the 75~80 pound mark on torque if you hear a loud "crack" noise and also feel the torque wrench quickly move several degrees - then you keep tightening and the same thing happens a second and/or third time..

...and eventually get 89~90 lbs torque, then
The washer is flattened and needs to be replaced. The clutch bolt will come loose, losing about 20 lbs torque "in progress" while you are driving the sled.
The vehicle is in the air and lands, or wheelie or catwalk and the vehicle lands and on full throttle or the track spins and hooks up - that "cushion" can shear in 1/2 just like your picture.
You need a new governor cup - thee end !
What's more economical? New gov cup? or Helicopter ride?
